HESS Pilot Feedback Form

We appreciate your feedback on the Health Equity Summary Score (HESS) HESS Dashboard, and its supplementary materials! Your responses will be used to inform revisions to the dashboard and supporting materials in preparation for a production release in early 2023, as well as ongoing score and dashboard improvement efforts.

You are welcome to submit your feedback anonymously; however, we request that you please provide your Contract ID if any of your feedback may be specific to your plan’s data. If you are open to being contacted with additional follow up questions as we review your feedback, please submit your name and email address.
