NSPS Care Delivery Network - Tier C Capabilities Input Survey

The National Special Pathogen System of Care (NSPS) leadership are asking for your feedback and expertise to help us define minimum operational capabilities for facilities in a national Care Delivery Network to support initial assessment, provision of care, and diagnostics. You were nominated to receive this survey by your RESPTC leader or NETEC given your expertise and perspective to offer to this subject.
 
Facilities in the NSPS Care Delivery Network are envisioned to be a part of a national, tiered, coordinated response network for patients suspected of or infected by a special pathogen. The Tiers range from Tier A (the most specialized) to Tier D (the most accessible). The NSPS Care Delivery Network leadership are focused on building out the minimum viable capabilities for Tier C (see additional details in graphic below).

In this survey, we share initial ideas proposed for an overview of Tier C and its minimum viable capabilities. We would greatly appreciate your feedback, specifically what you would add, edit, or remove from the descriptions listed.
